Case Manager
EDUCATION/EXPERIENCE:
- Bachelor’s degree from an accredited college or university;
- Two years of experience in job development, job placement, employment counseling, or job seeking skill training and motivation;
- experience in social services field;
- must have access to a vehicle;
- excellent organizational, interpersonal, written, and verbal communication skills;
- ability to perform comfortably in fast-paced, deadline-oriented work environment;
- ability to successfully execute many complex tasks simultaneously; and ability to work as a team member, as well as independently.
JOB RESPONSIBILITIES:
- Tracks client attendance and participation in work activities.
- Develops close, trusting, ongoing relationships with clients and community.
- Updates customer status promptly and coordinates actions with others.
- Schedules appointments, reviews employment goals and makes individual job matches.
- Utilizes group discussion and individual skill practice to build client self-confidence.
- Counsels clients regarding barriers to employment and workshop attendance.
- Will travel to customer’s home or work as situation calls for.
- Ensures all customers complete a master employment application, custom resumes created, and have practiced interviewing techniques.
- Ensures that customer participation in job search is in accordance with the written employability plan.
- Verifies job searches and training attendance.
- Assists customers to resolve scheduling problems and plan their job search activities.
- Will be available to customers on weekends/evenings as needed (cell phone will be provided to PA).
- Monitors and counsels customers concerning the effectiveness of the written employability plan.
- Prepares and submits reports as requested by management.
- Performs other duties as may be assigned by management.
Licensed Clinical Social Worker
EDUCATION/EXPERIENCE:
Master’s degree in Social Work from an accredited school of social work; two years of post Master’s experience; license from the State of Maryland as a Licensed Certified Social Worker – Clinical (LCSW-C); clinical experience in counseling, substance abuse, domestic abuse, and the like; ability to facilitate support groups; excellent organizational, interpersonal, written and verbal communication skills; ability to perform comfortably in a fast-paced, deadline-oriented work environment; ability to successfully execute many complex tasks simultaneously, and ability to work as a team member, as well as independently. Knowledge of Microsoft Suite, Word, Excel and Access.
JOB RESPONSIBILITIES:
- Provides one-on-one counseling to customers
- Prepares an individualized employment plan that details a customer’s employment goals, objectives, and timeframes
- Facilitates support groups for customers of the Employment and Support Services Project, such as job retention, parenting skills, recovery groups and anger management groups.
- Provides linkages to the community-based service network and works with Employment Services Specialist to ensure the appropriateness of their referrals.
- Identifies barriers to successful employment placement and self-sufficiency and develops a plan to address each issue, assists in the implementation of the plan, and participates in the follow-up of the implemented plan.
- Serves as the primary resource for crisis intervention of a short-term nature and duration.
- Works with customers of the who are at risk of being terminated from employment due to behavioral issues.
- Facilitates referrals for ongoing individual counseling.
- Conducts an assessment of a participant’s employability, including but not limited to, work history, skills, aptitude and transportation and childcare resources
- Performs other duties as may be assigned by management.
- Conduct workshops as assigned by Project Manager
- This is a 40 hour per week position. Each work day is 8 hours.
- Work with short term disabled customers:
a. Get medical extended for one year.
b. Move into some combination of countable activity hours.
c. Send letter to existing customers.
d. Invite them in for an assessment.
e. Assess new customers at the point they are assigned disability status.
f. What to Get From Customers:
i. Sign a release of information.
ii. Allow LCSW to contact physician.
g. Work with clients individually.
h. Work with customer and physician.
i. Establish an appropriate activity agreement.
j. Use assessment tool approved by DHHS.
k. Customers assigned
i. Can be assigned by the social worker.
ii. Customers can self declare their hours.
- Determine what outside activities the customer can be involved in.
a. Volunteering
b. Enrollment in vocational rehabilitation for no more than 3 months in a 2 year period.
c. Two day a week activity in combination with another activity.
- Referring new and existing customers to the social worker
a. Refer clients with multiple barriers.
b. Refer clients who cannot be successful in the employment services program.
- Triggers for referrals
a. Multiple barriers identified, client unresponsive to support.
b. Non compliant.
c. Long term repeat interviewers unsuccessful in acquiring employment.
d. Suspicion of sabotaging behavior.
e. Problem behavior in the employment services office.
- Expectations of LCSW
a. Provide short term crisis intervention counseling.
b. Use as part of the customers work activity
c. Refer to outside resource team or other outside partners.
d. Work with customers suspected of having mental health problems or learning disabilities.
e. Make referrals to appropriate agencies.
f. Establish strong relationships with service providers for diagnosis and treatment referrals.
g. Establish contact with long term disabled customers.
h. Send letters.
i. Review current status of medical and SSI application on appeal.
j. Participate in staffing as appropriate.
k. Facilitate support groups for customers related to parenting skills, anger management, overcoming obstacles.
l. Other topics as identified.

Requirements:
Must have excellent speaking and listening comprehension in English and Spanish, and excellent customer service skills.
Schedule varies greatly and is on-call as needed and requires someone with maximum flexibility, Monday through Friday between the hours of 8:00 am and 7:00 pm in Prince George’s County, Maryland. Hourly rate is based on experience, up to $19/hour.

COMMUNITY HEALTH NURSE
NATURE OF WORK: This position provides community health nursing services to families in the Anne Arundel County. This Community Health Nurse will be responsible for the administration of immunizations at the center as well as for special immunization clinics, including flu, at other sites, pregnancy testing and venipuncture as necessary. Duties also include assisting families with Maryland Children’s Health Insurance applications, education and outreach to individuals and health care providers,
and referral to other community resources as appropriate.
MINIMUM QUALIFICATIONS: A bachelor’s degree in nursing from an accredited college or university. Two years of experience as a registered nurse or a bachelor’s degree in a related field from an accredited college or university and one year of experience ads a registered nurse may be substituted for the required education.
LICENSE: Candidates must possess a current license as a Registered Nurse from the Maryland State Board of Nursing.
A criminal background check will be conducted which includes fingerprinting and also a driving record check. This position is located in Anne Arundel County, Maryland.
